Definition
Anaemia is a condition caused by reduced haemoglobin (Hb) concentration in the blood (i.e., decreased number of red blood cells). This indicator measures the prevalence of anaemia in children 6-59 months and women of reproductive age (15 to 49 years), broken down by total, mild, moderate or severe according to the WHO classifications for the management of nutrition in major emergencies (2000).
Rationale
Anaemia is highly prevalent globally, disproportionately affecting children and women of reproductive age. Anaemia is associated with poor cognitive and motor development, and work capacity. Among pregnant women, iron deficiency anaemia is also associated with adverse reproductive outcomes such as preterm delivery, low-birth-weight infants, and decreased iron stores for the baby, which may lead to impaired development. Iron deficiency is considered the most common cause of anaemia but there are other nutritional and non-nutritional causes. Blood haemoglobin concentration is used to diagnose anaemia and it is affected by many factors, including altitude (metres above sea level), smoking, trimester of pregnancy, age and sex. When blood haemoglobin concentrations are used in combination with other indicators of iron status, they provide information about the severity of iron deficiency. The prevalence of anaemia in a population can be used to classify the public health significance of the problem (WHO, Global Health Observatory).
In forced displacemen settings, the primary objective of this indicator is to measure the prevalence of anaemia in children aged 6-59 months and in non-pregnant women of reproductive age between 15-49 years. The secondary objective is to determine enrolment into antenatal care (ANC) and coverage of iron-folic acid supplementation in pregnant women.
Method of computation
Anaemia prevalence is calculated by dividing the number of people (of interest) with total number of eligible people with a valid haemoglobin measurement.
Haemoglobin concentration in children 6-59 months and women 15-49 years is taken from a capillary blood sample from the fingertip and recorded to the closest gram per decilitre (g/dL) or gram per litre (g/L). The level of g/dL is used to determine the category of anaemia. In children 6-59 months and pregnant women, the follwing cut-offs as defined by the WHO are used: total <11.0, mild 10.0-10.9, moderate 7.0-9.9, and severe <7.0. In non-pregnant women of reproductive age (15-49 years), the following cut-offs as defined by the WHO are used: total <12.0, mild 11.0-11.9, moderate 8.0-10.9 and severe <8.0. If severe anaemia is detected, the measurement may be repeated for confirmation purpose.
In UNHCR surveys, a child's age may be determined using the exact date of birth or month and year of birth calculated using local events calendar should be recorded for children. For women, the reported age (rather thant he exact date of birth) and reported pregnancy status is used.
